Waiver & Release Agreement

1. Assumption of Risk

I understand that participation in soccer training involves inherent risks including physical injury. I voluntarily allow my child to participate in the Himalaya Baltimore 2026 Kids Soccer Training Camp.

2. Medical Authorization

I authorize emergency medical treatment if needed and accept full responsibility for any resulting medical expenses.

3. Release of Liability

I release Himalaya Baltimore, its officers, coaches, volunteers, sponsors, and affiliates from any liability arising from participation, except as prohibited by applicable law.

4. Indemnification

I agree to hold harmless the organization from any claims, damages, or costs related to my child's participation in the camp.

5. Photo & Video Release

I authorize Himalaya Baltimore to use photographs and videos of my child for promotional and organizational purposes without compensation.

6. Camp Rules

My child agrees to follow all camp rules and guidelines, and to demonstrate good sportsmanship at all times.

7. Insurance Acknowledgment

I understand and acknowledge that Himalaya Baltimore does not provide medical insurance coverage for participants. I am responsible for maintaining my own family's health insurance.
